I understand that Core Health Colleagues, Inc., DBA CoreHealthco.com, assumes no responsibility for injuries or illness which I may sustain as a result of any physical condition resulting from participation in any athletic activities, sport programs, use of any recommended equipment, exercise, diet/nutrition or other activities outlined in content, video programs, slide shows or materials associated with the Core Health Colleagues programs. I expressly acknowledge on behalf of myself and my heirs, that I assume the risk for any and all injuries and illnesses that may result from participation in the StartRightNow programs and activities. I hereby release and hold harmless Core Health Colleagues, its agents, servants, and employees, from any claim for injury, illness, death, loss, or damage which I may suffer as a result of my participation in these activities. I hereby certify that I am in normal medical condition and capable of safe participation in the StartRightNow programs.
